REPORTING ERRORS AND RECOMMENDING IMPROVEMENTS
You can help improve this manual. If you find any mistakes, or if you know of a way to improve
the procedures, please let us know. Reports, as applicable by the requiring service, should be
submitted as follows:
(a) Army (A)
- The preferred method is to submit your DA Form 2028 (Recommended
Changes to Publications and Blank Forms) through the Internet on the TACOM Unique
Logistics Support Applications (TULSA) website. The Internet address is
https://tulsa.tacom.army.mil. Access to all applications requires CAC authentication, and you
must complete the Access Request form the first time you use it. The DA Form 2028 is
located under the TULSA Applications on the left-hand navigation bar. Fill out the form and
click on SUBMIT. Using this form on the TULSA website will enable us to respond more
quickly to your comments and better manage the DA Form 2028 program. You may also
mail, e-mail, or fax your comments or DA Form 2028 directly to the U.S. Army TACOM Life
Cycle Management Command. The postal mail address is U.S. Army TACOM Life Cycle
Management Command, ATTN: AMSTA-LCL-IM / TECH PUBS, MS 727, 6501 E. 11 Mile
Road, Warren, MI 48397-5000. The e-mail address is tacomlcmc.daform2028@us.army.mil.
The fax number is DSN 786-1856 or Commercial (586) 282-1856. A reply will be furnished
to you.
(b) Marine Corps (MC)
- Submit notice of discrepancies or suggested changes on a NAVMC
10772. The NAVMC may be submitted using either of the following:

NAVMC 10772 program should be reported by calling DSN 567-7628 or DSN 567-5017
(Commercial number is (229) 639-7628 or (229) 639-5017).
b. The alternate method of submittal does not require a Common Access Card (CAC) to
access the form. Click on http://navalforms.daps.dla.mil/web/public/forms, select the "Keyword
Search" button, enter "10772" in the Search Criteria Box. Under "type" click on download page
button. Click on PDF icon.
Enter user data in the appropriate fields. Must have users contact
information block filled with Unit address and telephone number. Click on the “Envelope” icon
in the tool bar. Select "Send Copy,” click on "OK". When the PDF document is created, an
Outlook Email screen will open with the .PDF as an attachment.
On the TO: line type
SMB.LOG.Tech.Pubs.fct@usmc.mil. In the body of the email, type any additional information
you wish to provide. Click “SEND”.
c. A reply will be furnished to you.
