B-1. DA Form 2402
a. Use of Form 2402. The DA Form 2402 must be
lled out and attached to any and all part(s)
removed from the vehicle. Refer to Figure B-1 for an example of a completed DA Form 2402.
b. Block 11.
Enter the Freightliner chassis serial number of the vehicle found on the vehicle
data plate.
