REPORTS
a.
When the vehicle is recovered/evacuated for permanent standard repair, and DA Forms 2404 and 2407 are
used, the notation "BDAR" shall be added in the space provided for description of deficiencies.
b.
DA Pam 738-750 provides for disposition of DA Form 2404 and copy number 3 of DA Form 2407.
When
"BDAR" is noted on these forms, they shall be mailed to: WL/FIVS-SURVAC, BLDG 45, 2130 8th Street, Suite I, Wright
Patterson AFB, Ohio 45433-7542.
The Information on these forms will provide data for designing vehicles to be less
susceptible to combat damage and easier to repair when damaged.

The battlefield damage assessment/repair forms used are listed in Chapter 2.
