or destroyed. No problem, jUst folLow the instructions in para 4²³b (2) a of T´
38¸750¹ ºill out a permanent copy of the ±cceptance Report.
ºill in as much of the original informatIon as you can find. Put "»NK¼ for
"Unknown¼ in the blocks you can't fill.
Write "New Form ¶nitiated¼ and "Permanent ½ogbook Copy¼ and the Julian
date you fIll out the form in the Remarks Block.

¶f the eqUipment arrives from sources other than procUrement without an
±cceptance Report, contact the unit who sent you the item to see if they stiLL
have the form or information.
¶f they don't, you can't be sure an ±cceptance Report was turned in, so yoU
need to make up a D± ºorm 2408²³ Gain Report.

When the info hits Áhe computers at ½exington, your Gain will slip right in
Like an ±cceptanc Report. ¶f the info's already there, the form bounces with no
sweat. ¶f info's not there, your Gain Is accepted.
